Requirements for Life Certificate: -

 Self-Declaration Form A (2copies)

 The person should visit SGIVS Global LLC, Al Qurum Muscat with his Original Passport and its copy

 Photocopy of valid ID card / residence card original and copy

 Two recent passport size photographs

Requirements for Life Certificate: -

 The person should visit SGIVS Global LLC, Al Qurum Muscat, with his Original Passport and its copy

 Photocopy of valid ID card / residence card original and copy

 Two recent passport size photographs

Fee :   Fee : Nil (Gratis), SGIVS service charges may apply 

Submission time : 0730 hrs to 1530 hrs 

Note: 

Applicant may contact Indian Embassy number 98282270 for facilitation if they find it difficult to visit SGIVS Global, Muscat for applying for Life Certificate.
