New Passport in lieu of Damaged Passport

Document required

  • A printout of duly filled online application form available at https://embassy.passportindia.gov.in/ should be submitted at BLS Centre. All particulars in the application should tally with those in the current passport.
  • Damaged passport, in original, along with a photocopy.
  • A copy of ID card / residence card.
  • Two white background photographs. (Please refer to the photo specification mentioned in general guidelines)
  • Police Report for loss of passport and a letter issued by the local immigration subsequent to the police report (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage)
  • A letter addressed to the Embassy, from the sponsor/employer (from parent in case of a minor) of the applicant on the company's letterhead explaining circumstances leading to damage of the passport. One photo of the applicant should also be pasted on the letter. (This document is not required if there is only a minor damage).
  • Original with a copy of one or more of the photo-affixed identity cards issued by Indian authorities. Original will be returned after verification.
  • A declaration for obtaining the new passport in lieu of lost/ damaged passport as per Annexure F

Note:

  • If the passport has expired for more than three years, Prior approval is required from Consular Officer.
  • All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at BLS Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Shinas, Duqm Ibri and Dibba.
  • This is also applicable for minors and new born babies.
