New Passport in lieu of Lost Passport

Document required

• As per instructions in general guidelines, printout of duly filled in online application form available at https://embassy.passportindia.gov.in/ should be submitted at SGIVS Global LLC in Muscat or its collection centres at Salalah, Sohar, Sur, Nizwa, Buraimi, Ibri, Ibra, Duqum, Khasab & Barka Centre. All particulars in the application should tally with those in the current passport.

• Three white background photographs. (Please refer to the photo specification mentioned in general guidelines)

• Photocopy of the lost passport.

• Copy of ID card / residence card.

• Police report for loss of passport and a letter issued by the local immigration subsequent to the police report.

• A letter addressed to the Embassy, from the sponsor/employer (from parent in case of a minor) of the applicant on the company's letterhead intimating about the loss of the applicant's passport and explaining circumstances leading to the loss of the passport. Company Registration copy including authorized signatory paper or any other identity proof of the sponsor.

• Original with a copy of one or more of the photo-affixed identity cards issued by Indian authorities. Original will be returned after verification.

• A declaration for obtaining the new passport in lieu of lost/ damaged passport as per Annexure F

Note:

• If the passport has expired for more than three years, Prior approval is required from Consular Officer.

• All applicants seeking passport or passport related services are required to be physically present at the time of making application in the Sultanate of Oman and should visit in person at SGIVS Global LLC in Muscat or its collection centres at Salalah, Sohar, Sur, Nizwa, Buraimi, Ibri, Ibra, Duqum, Khasab & Barka Office in Muscat or its mobile collection centres located at Sur, Sohar, Nizwa, Salalah, Buraimi, Khasab, Shinas, Duqm Ibri and Dibba.

• This is also applicable for minors and new born babies.
